The registration process requires the submission of an application form, a character certificate, and a sworn affidavit.
Application Form for Registration of Service Provider
Applicants are required to fill out the following details:
Personal Details:
- Name (in English and Hindi)
- Mobile Number
- Email Address
- Gender (Male, Female, Transgender)
- Marital Status (Married, Unmarried, Widower, Widow)
- Date of Birth (Day, Month, Year)
- Father's Name (in English and Hindi)
- Religion (Hindu, Muslim, Sikh, Christian, Jain, Buddhist, Parsi)
- Category (General, Other Backward Class, SC, ST)
- Caste Certificate Number
- District
- Office
- Unique Identification Type and Number
Address Details:
- Photograph required
- Correspondence Address (State, Area - Rural/Urban, District, Tehsil, Block/Municipal Corporation/Municipality/Nagar Panchayat, Revenue Village/Ward/Mohalla)
- PIN Code
- Permanent Address (if different from correspondence address)
Skill Details:
- Applicants can list multiple skills.
- For each skill, they must specify if they have completed a vocational or technical course, the name of the recognition body, the institute's name, and the training status (Completed/Ongoing).
Work Area:
- Applicants must specify their preferred area of work (State, Area - Rural/Urban, District, Tehsil, Block, etc.)
Work Schedule:
- Working Hours: Choose from various time slots (e.g., 8:00 AM - 10:00 AM, 10:00 AM - 12:00 PM, etc.).
- Working Days: Choose from Monday to Sunday.
Character Certificate
A character certificate proforma is included, which requires the following information:
- Orders/Instructions Details
- Applicant's Name
- Father's/Husband's Name
- Age
- Educational Qualification
- Occupation
- Permanent Address
- Temporary Address and Phone Number
- Details of criminal cases at the police station, if any.
- General Reputation
- Clear mention of the applicant's conduct and character.
- Confirmation of no adverse entry against the applicant in Police Station Register No. 8.
- Any other details.

Applicants must attach the following documents:
- Photocopy of Identity Card
- Photocopy of Skill Certificate
- Photocopy of Police Verification Certificate
- Photocopy of Educational Qualification Certificates
- Original Affidavit
Affidavit for Registration in Sewamitra Application
An affidavit must be submitted on a Rs. 10 non-judicial stamp paper, attested by a Notary/Oath Commissioner. The declarant must affirm the following:
- They have complete knowledge of the skills they have registered for.
- They will be solely responsible for any financial dispute or accident that occurs during their work. The Employment Department will have no liability.
- All information provided in the application form is correct and no facts have been concealed.
- The statements in the affidavit are true.
