Proforma For Medical Certificate – Only For Ex-Servicemen (ExSM) Candidates Shortlisted For CBAT Against The Posts Of Station Master

This notice, issued by the Ministry of Railways, Railway Recruitment Boards, under CEN 05/2024 (For Recruitment of various NTPC Graduate Posts), provides a proforma for a Medical Certificate.

Important Instructions for Ex-Servicemen (ExSM) Candidates Shortlisted for CBAT for Station Master Posts

  1. In continuation to the notice dated 16.12.2025, the proforma for Medical Certificate, to be obtained by ExSM candidates aged 35 years and above from an Eye Specialist, is enclosed. This certificate is mandatory for candidates appearing for the Computer Based Aptitude Test (CBAT) for Station Master posts.
  2. This specific proforma is applicable only if an ExSM candidate's age is 35 years and above as on the date of their medical fitness test. If the candidate's age is less than 35 years, the proforma as per Annexure-VI of CEN No. 05/2024 is applicable instead.
  3. Shortlisted ExSM for CBAT must produce the Medical Certificate in original at the time of entry for CBAT. Failure to do so will result in not being permitted to appear in the CBAT.
  4. Candidates are strongly advised to refer only to the official websites of Railway Recruitment Boards (RRBs) for the latest updates on the recruitment process. Please exercise caution and do not be misled by unauthenticated or unofficial sources.
  5. Candidates are warned to beware of individuals (touts) who may attempt to misguide them with false promises of appointment for jobs through illegal means. RRB selections are strictly based on performance in the Computer Based Test (CBT) and the overall merit of the candidates.

Medical Certificate Proforma Details

This proforma is to be completed by an Eye Specialist for Ex-Servicemen candidates aged 35 and above applying for the posts of Station Master.

  • Candidate Information: The proforma includes space for the candidate's name (Smt./Shri/Kumari) and details of the eye examination.
  • Vision Standards for Station Master (Class A-2):
    • Distant Vision: 6/12, 6/12 or 6/9, 6/18 with or without glasses. Naked eye vision must not be less than 6/60. The power of the lens used should not exceed 6 Dioptres (D).
    • Near Vision: The combined near vision, with or without glasses, should demonstrate the ability to read ordinary print. Where reading or close work is required, the combined near vision, with or without glasses, should be Snellen 0.6 (Sn. 0.6).
    • Colour Vision: Ishihara test results must be Normal.
  • Certification: The Eye Specialist is required to certify that the candidate's vision/colour vision conforms fully to the prescribed standards.
  • Photograph and Signature: A self-attested recent passport-size photograph (not more than one month old) of the candidate must be pasted, and the candidate's signature must be placed below it.
  • Specialist's Details: The proforma includes fields for the Eye Specialist's signature, name, registration number, place, and date.

Note: This specific certificate is exclusively applicable if the Ex-Serviceman candidate's age is 35 years and above on the date of the medical fitness test. For candidates less than 35 years of age, the proforma as per Annexure VI of CEN No. 05/2024 is applicable.
