Instructions for Filling the Identity Profile

This document provides detailed instructions for filling out the 'Identity Profile' as part of the Universal Registration process on the Union Public Service Commission (UPSC) online portal. Users are advised to read the general instructions before proceeding.

Accessing the Identity Profile Module

  1. Log in to the UPSC online portal using valid credentials.
  2. Click on the 'Universal Registration' link from the navigation bar.
  3. Read the instructions for Universal Registration.
  4. Click on "Proceed for Universal Registration".
  5. Fill all mandatory fields on the Identity Profile form.
  6. This profile can be navigated to later before locking the Universal Registration.

Entering Information in Identity Profile

Users must accurately enter their name as follows:

  1. Enter your First Name.
  2. Enter Middle Name (if applicable) and Last Name (if applicable).
  3. Confirm the First Name, Middle Name, and Last Name.

Name Entry Examples:

  • Three or more parts: First Name - Middle Name - Last Name (e.g., Suresh Prasad Singh -> First: Suresh, Middle: Prasad, Last: Singh)
  • Two parts: First Name - Last Name (e.g., Rohit Kumar -> First: Rohit, Last: Kumar)
  • One part: Enter in First Name (e.g., Divya -> First: Divya)

Additionally:

  • Indicate if the entered name matches the 10th/Matriculation/Equivalent Examination Certificate by selecting 'Yes' or 'No'.
  • If 'No' is selected, provide the name exactly as per the 10th Class/Matriculation/Equivalent Board Examination Certificate.

Name Matching Scenarios

  • Case 1: Name Matches (Select "Yes") The name details entered will be auto-fetched.
  • Case 2: Name Doesn't Match (Select "No") Users must enter their name as printed on the 10th Class/Matriculation/Equivalent Board Examination Certificate. Additional details regarding the change of name must be provided, including:
    • State from which Gazette Notification is issued.
    • District from which Gazette Notification is Issued.
    • Gazette Notification number.
    • Gazette Notification Issued on (dd/mm/yyyy).
    • Document Upload: A valid and readable copy of the change name Gazette document (PDF, 50-300KB, filename: "name_change.pdf") must be uploaded.

Providing Additional Identity Details

  1. Select 'Gender' from the dropdown menu.
  2. Enter 'Date of Birth' in dd/mm/yyyy format and confirm it.
  3. Enter 'Father's Name' and confirm it.
  4. Enter 'Mother's Name' and confirm it.
  5. Enter 'Guardian's Name' and confirm it, if applicable (optional).

Review and Submission

  1. Carefully review all entered information for accuracy.
  2. Click the 'Next & Review' button to proceed.
  3. Make any necessary corrections before final submission.

Check Details on Review Page

Thoroughly review the application details. Use the 'Edit' button for modifications. Once all details are verified and accurate, click 'Submit' to finalize the identity profile module.

After Successful Submission of "Identity Profile"

Upon successful submission of the "Identity Profile", proceed to fill out the "Matriculation Board Profile" and continue completing subsequent sections as per the guidelines.
